1. If you were not a member of Hallstrom in the past, were you involved in another homeschool support group or co-op? If yes, what teaching or other responsibilities did you fulfill in that group? 2. Are both parents Christians? Briefly share something about your Christian walk(s). 3. Briefly describe your and your spouse's approach to discipline with your children. 4. If you or your child does not agree with an item in the Hallstrom Code of Conduct or a Rockford FIRE sports policy (if applicable), how would you address that with your child? 5. How important are academics in your family's approach to home schooling? Please explain. 6. Have your older children (grades 7-12) come to faith in Christ? How well do they receive instruction, and how do they respond to authority and discipline? 7. Have you removed your child(ren) from a traditional school setting to be home schooled? If so, when, and for what reason(s)? 8. Have any of your children ever been, or are they currently, suspended or expelled from a public or private school? If so, please explain. 9. Have any of your children ever been disciplined by school authorities for or been involved in the use of drugs, alcohol, firearms, or weapons at school? If yes, please explain. 10. Are any of your children currently taking medication for or been diagnosed with ADD, ADHD, or any other physical, behavioral, or mental disability or disorder? If yes, please explain. I understand and agree that I am responsible for completing my assignments on time and for doing them to the best of my abilities because that is my “good and faithful service” to the Lord.
